Request Forms

The library offers a variety of services that are available through a convenient form submission. If you encounter any problems with these forms, you can contact our Digital Services Specialist.

Book a Room

The library has space available to the Rollins community to use for classes and events. Learn more about the available spaces here before reserving a room through the Virtual EMS system.

Questions about room reservations can be directed to Scheduling and Event Services or our Office & Facilities Coordinator.

Database Recommendation Form

Current students, faculty, and staff are encouraged to use this form to recommend databases for the library to subscribe to.

All completed database recommendation forms will be sent to the Electronic Resources Librarian and the department’s liaison librarian, who will contact the submitter to begin a formal review and possible trial. A trial will allow the library to evaluate the database and gather valuable feedback from the campus community in order to make an informed purchasing decision.

Faculty Reserve Form

If you are teaching a class and need to put materials on reserve, you can do so with this form.

Our Circulation and Information staff can assist with any reserves questions.

Game Recommendation Form

Current students, faculty, and staff are encouraged to use this form to recommend games for the library to purchase.

Interlibrary Loan

If the Olin Library doesn’t have access to an article, book, or film, we can attempt to get the item for you via Interlibrary Loan from another library. Books and films are delivered to the library for you to pick up at the Circulation and Information Desk, and articles are delivered to you electronically in .pdf format.

Your Interlibrary Loan login information is your regular FoxID and password. Read the Interlibrary Loan guide for a step-by-step process.

All completed journal recommendation forms will be sent to the Head of Collections, Acquisitions Coordinator, and Electronic Resources Librarian.
